Job Summary

To work as a member of the multidisciplinary team, within a dedicated area. The post holder will carry out reception, clerical and any relevant non-clinical duties, which assist nursing and medical staff and patients.

 

System Knowledge

Duties

  • Complete the necessary documentation and data inputs for the admission, transfer and discharge of patients.  Using computer systems to check the ward bed state and record all patient activity, liaising with Admissions to ensure that this data is correct.
  • Whilst the patient is on the ward, create and maintain Patient Case Notes following procedures, regularly check patient records to ensure that all loose papers, history sheets, investigation reports have been appropriately secured and filed in the correct order.  If the case notes leave the ward with the patient during investigations, ensure that the notes are returned.
  • When a patient is transferred or discharged, ensure that the case notes and X-Rays are complete and all loose papers have been filed in order.  When the patients are transferred or discharged record movements in the case note tracking system.
  • Ensure late investigation results are married up with any case notes still on the ward.  Alternatively, identify where notes are using Case Note Tracking and dispatch late results either to this location or to the relevant Consultant's medical secretary.
  • At ward level keep the necessary records of the death of a patient ensuring that the notes and X-Rays are complete and in order before being passed on to the Histopathology secretary.  Apply adhesive "Patient Deceased" notice on the case notes.  Record death on relevant system.  Complete and dispatch the notification of death letter to the relevant General Practitioner.  Out of courtesy, a telephone call should be made to the GP practice.
  • Provide a courteous reception and telephone service to patients, visitors, carers and other users.
  • Assist the ward team in providing a welcoming environment for all newly admitted patients and their families and friends.  Facilitate communication with all the care professionals, service departments and relevant parties, receiving and relaying messages and other information.
  • Assist in the organisation of visits from Social Workers, Community Nurses, Respiratory Nurses, Macmillan Nurses, Chaplains or any other member of the multi-disciplinary team on instruction of the nurse in charge of the ward.
  • Inform the Dietician and Kitchen when a relevant patient has been discharged home and liaise with Kitchen staff to ensure the patient is seen in the event of catering complaints.
  • Ensure that requests for tests and investigations are dispatched and/or delivered promptly to special departments e.g. X-Ray, Medical Physics, etc., and that results are conveyed to nurses and doctors as soon as possible.
  • Make the necessary arrangements for Out Patient appointments, ensuring whenever possible that the patient is aware of any outstanding appointment prior to discharge.  Arrange transport to and from the hospital as required, referring clinical decisions to nurse in charge of the ward.
  • Assist the patients with any reasonable request (i.e. helping with the telephone, posting letters).  Deal helpfully with any enquiries from the patients' relatives and friends.  Provide information and assistance in a co-operative manner.
  • Complete requisition for provisions, stores and stationery.  Put supplies and stock away in appropriate cupboards.  Telephone Maintenance with requests for repairs and keep a record of when works are ordered and carried out and follow up as required.
  • Assist in the day to day running of the ward by performing general housekeeping duties as and when required by the nurse in charge.
